Fluoroquinolone (FQ)-associated tendinopathy and myopathy are uncommon but well recognized complications of the use of this class of antibacterial agents. The case of a 63-year-old previously asymptomatic female patient who developed severe left shoulder tendinopathy after surreptitiously doubling the prescribed dose of levofloxacin for the treatment of community-acquired pneumonia is reported here. Surgical stabilization with suture anchors and subacromial decompression were needed. With the expanded use of fluoroquinolones for the treatment of community-acquired respiratory infections and reports of tendon injury linked to the use of these agents, we reviewed the literature to investigate the frequency and strength of this association. Ninety-eight case reports were available for review. The incidence of tendon injury associated with fluoroquinolone use is low in a healthy population but increases in patients who have renal dysfunction, who are undergoing hemodialysis, or who have received renal transplants. Pefloxacin and ciprofloxacin were most frequently implicated, but tendon injury was reported with most fluoroquinolones. The median duration of fluoroquinolone treatment before the onset of tendon injury was 8 days, although symptoms occurred as early as 2 hours after the first dose and as late as 6 months after treatment was stopped. Up to one-half of patients experienced tendon rupture, and almost one-third received long-term corticosteroid therapy. Tendon injury associated with fluoroquinolone use is significant, and risk factors such as renal disease or concurrent corticosteroid use must be considered when these agents are prescribed.

The pathophysiology of drug-induced tendinopathy and rupture is poorly. with bilateral Achilles tendinopathy secondary to fluoroquinolone antibiotic use. Some of the more common ones are Levoquin levofloxacin, Ciproxin ciprofloxacin other common drug names are Baloxin and Tequin, and common chemical names are enoxacin, lomefloxacin and norfloxacin. In the literature many side effects have been documented including tendonitis and subsequent tendon rupture. As ciprofloxacin and other fluoroquinolones are prescribed more frequently. Researchers1,16 have also found fluoroquinolone-induced tendinopathy can.

Nearly all quinolone antibiotics in use are fluoroquinolones, which contain a fluorine atom in their chemical structure and are effective against both Gram-negative and Gram-positive bacteria. One example is ciprofloxacin, one of the most widely used antibiotics worldwide. Fluoroquinolones are often used for genitourinary infections and are widely used in the treatment of hospital-acquired infections associated with urinary catheters. In community-acquired infections, they are recommended only when risk factors for multidrug resistance are present or after other antibiotic regimens have failed. However, for serious acute cases of pyelonephritis or bacterial prostatitis where the person may need to be hospitalised, fluoroquinolones are recommended as first-line therapy. Due to people with sickle-cell disease being at increased risk for developing osteomyelitis from the Salmonella, fluoroquinolones are the "drugs of choice" due to their ability to enter bone tissue without chelating it, as tetracyclines are known to do.

Fluoroquinolone-induced tendinopathy is a drug-induced dose-dependent toxic effect of these agents.10,11 Chhajed et al.12 found Achilles tendon rupture occurring at a lower total dosage of ciprofloxacin than other tendon disorders, suggesting an idiosyncratic response to the drug in some patients. Everyone knows drug prices in the United States are out of control. It’s no wonder Americans turn to alternative sources, such as buying online, to get the medications they need. Especially when those medications can be found 75-90% cheaper than buying at the local pharmacy. Well via the New Yorker: Is “generic Viagra” from mail-order pharmacies legit? For the most part, their answer is very one-sided and misses out on some important details. So while their answer may have left us hanging, pardon the pun, we thought we’d try giving a more accurate answer. No other drug, generic or otherwise, is as sought after online as Pfizer’s signature drug, Viagra. Unfortunately for some, this can also mean acquiring potentially harmful counterfeits if purchased from less than reputable sources.
